What does getting sick in Aruba cost a visitor?
Aruba's medical story is the honest small-island one: one hospital serves the island, and the complex case flies out. The hospital is decent, the island is safe and close to the US, and none of that changes what happens when a case outgrows what one island facility can do.

The flight off the island
Aruba runs one hospital, the island's own, plus clinics and an urgent-care tier serving the resort strip. For the everyday, that is comfortably enough: visitors are seen quickly, English is spoken everywhere, and you are billed as a visitor, with payment or proof of cover expected around admission. The structural limit is capacity, not quality: a single island facility cannot hold every specialty, so the complex case flies out, to Colombia, to the US mainland, or elsewhere in the region depending on the case and the insurer. In the Dutch Caribbean generally, official US guidance prices that pattern plainly: evacuations in the region typically run to the tens of thousands of dollars. The flight, not the ward, is what a Aruba policy is really for.
One hospital, and it is fine for the everyday
For the problems a beach trip actually produces, sunburn gone wrong, an ear infection, stitches, a bad stomach, Aruba is easy. The island's hospital and the clinics serving the resort strip see visitors constantly, English is universal, and the care is decent.
If it looks serious, go to the hospital first and let the billing follow. You are a paying patient here: expect to show a card or proof of cover around admission and to settle or document everything before you leave, with reimbursement your problem unless your insurer direct-bills.
The ceiling is the flight
The limit is arithmetic, not competence: one hospital cannot staff every specialty for every emergency, so cardiac surgery, major trauma, complicated obstetrics and the like are stabilised and flown out. Colombia is close and capable, the US mainland is the other pole, and the destination is usually your insurer's call as much as the doctors'.
Official US guidance for the Dutch Caribbean puts the regional pattern in round terms: medical evacuation typically costs in the tens of thousands of dollars, paid by the patient or the policy. That is the one number-shaped fact worth planning around here, and it belongs to the flight.
Why there are no prices on this page
We would rather say this than pad the page. We did not source a published Aruban tariff for visitor care that we would put our name to, so no figure appears here beyond the qualitative regional evacuation pattern above. On a medical page a plausible-sounding number is not a small error, because it is something a person might act on while unwell.
The structure is enough to plan with: one hospital, everyday care handled, visitors billed, and a flight for the complex case. The evacuation line on a policy is the line doing the work.
What this means for your money
Aruba is a short-trip island, and short trips are where people skip cover, which is backwards here for the same reason it is in the Bahamas: the trip's length does not shrink the flight's price. The everyday you can pay for; the flight off the island you insure.
Save your insurer's assistance number, carry a card with headroom for the admission conversation, and keep every receipt. For paying on the ground, see the Aruba cash and cards guide.
Common questions
Quick answers.
How good is the hospital in Aruba?
Decent, and comfortably enough for the everyday problems a beach trip produces, with English spoken and visitors seen quickly. Its limit is capacity: one island facility cannot hold every specialty, so complex cases fly out.
Where do serious medical cases in Aruba go?
They are stabilised and flown out, to Colombia, the US mainland, or elsewhere in the region depending on the case and the insurer. That medically supervised flight is the largest realistic bill an Aruba trip can produce.
How much does a medical evacuation from Aruba cost?
Official US guidance for the Dutch Caribbean puts regional evacuations in the tens of thousands of dollars, paid by the patient or the policy. We publish no tighter figure because every flight is priced to its case.
Do I pay for medical care in Aruba as a tourist?
Yes, you are billed as a visitor, with payment or proof of cover expected around admission and reimbursement your problem unless your insurer direct-bills. Keep every receipt for the claim.
Do I need travel insurance for Aruba?
For the everyday, no argument is needed either way, it is small. The honest reason is the ceiling: a complex case flies off the island, and the evacuation line on a policy is what pays for that flight.
More on Aruba
The little money things nobody warns you about.
- Tipping in ArubaAruba leans closer to US-style tipping than most of the Caribbean, thanks to heavy US tourism. Check the bill for a service charge, and tip a bit more if none is added. Here is what you actually owe.
- Do I need cash in Aruba, or can I use cards and dollars?Short answer: you can do almost the whole trip on a card and US dollars, and you never need to exchange for florin. Here is how to pay, and the one habit that saves you money.
- How much does the Aruba ED Card cost, and do you need a visa?Short answer: no visa, and the form itself is free. What you pay is a 20 dollar sustainability fee, once per calendar year rather than per trip. The reason people end up paying much more is a set of copycat websites that buy ads, outrank the government portal, and charge to file the same free form. Here is what it actually costs, and how to make sure you pay only that.
- Getting around Aruba: taxis, buses and carsShort answer: taxis run on fixed government rates rather than meters, there is no Uber, and a rental car or the cheap public bus fills the gaps. Here is how each works and what it costs.
- Aruba hotel taxes and fees, explainedShort answer: the headline room rate is not what you pay. Between government levies and hotel charges, a nightly rate can climb by a quarter to a third. Here is exactly what stacks on top.
- Staying connected in Aruba.Aruba is 20 miles long, covered end to end, and the kind of place where most visitors spend the week between a hotel, a beach and a handful of restaurants that all have wifi. That makes the honest answer here uncomfortable for us: often, buy nothing.

Our own readThe one-hospital structure, the fly-out pattern for complex cases, and the billed-as-a-visitor position are structural facts consistent with official US guidance for Aruba and the Dutch Caribbean, stated qualitatively. The tens-of-thousands evacuation shape is the US Consulate's own regional characterisation, and we have kept it qualitative rather than turning it into a quote for any particular flight.
