Aruba runs one hospital, the island's own, plus clinics and an urgent-care tier serving the resort strip. For the everyday, that is comfortably enough: visitors are seen quickly, English is spoken everywhere, and you are billed as a visitor, with payment or proof of cover expected around admission. The structural limit is capacity, not quality: a single island facility cannot hold every specialty, so the complex case flies out, to Colombia, to the US mainland, or elsewhere in the region depending on the case and the insurer. In the Dutch Caribbean generally, official US guidance prices that pattern plainly: evacuations in the region typically run to the tens of thousands of dollars. The flight, not the ward, is what a Aruba policy is really for.
One hospital, and it is fine for the everyday
For the problems a beach trip actually produces, sunburn gone wrong, an ear infection, stitches, a bad stomach, Aruba is easy. The island's hospital and the clinics serving the resort strip see visitors constantly, English is universal, and the care is decent.
If it looks serious, go to the hospital first and let the billing follow. You are a paying patient here: expect to show a card or proof of cover around admission and to settle or document everything before you leave, with reimbursement your problem unless your insurer direct-bills.
The ceiling is the flight
The limit is arithmetic, not competence: one hospital cannot staff every specialty for every emergency, so cardiac surgery, major trauma, complicated obstetrics and the like are stabilised and flown out. Colombia is close and capable, the US mainland is the other pole, and the destination is usually your insurer's call as much as the doctors'.
